INTRODUCTION AND IMPORTANCE: Multisystem inflammatory syndrome in children (MIS-C) is a late-phase complication of coronavirus disease 2019 (COVID-19) featuring systemic hyperinflammation and multi-organ dysfunction. CASE PRESENTATION: We present the clinical features of a three-year-old girl who developed purpura fulminans and Acute Digital Necrosis and amputation while hospitalized for MIS-C. CLINICAL DISCUSSION: Several MIS-C-associated complications are being reported, including arterial and venous thromboembolic events that may lead to amputation of the affected limbs. CONCLUSIONS: This case emphasizes the need for to maintain a constant awareness of the possible thromboembolic MIS-C-related sequelae.
