Subject and method: Cross-sectional retrospective review with intervention/ adjustment. The laboratory receives external control samples in the form of blood smear from the Center for Medical Testing Quality Control - University of Medicine and Pharmacy in Ho Chi Minh City 6 times a year, each time 2 blood smear samples from 2019 to 2023, performs analysis and Send the results to the external control unit, the results evaluation sheet for each batch will be sent back after 10 days. The laboratory reviews and evaluates the results as inappropriate or acceptable, but needs to monitor, find the causes, propose intervention actions to correct errors, and monitor and evaluate performance after correction over the years. next and make recommendations to prevent potential risks. Results: Evaluate of laboratory is always at the high level with the "very good" rate increasing from 66.67%-100%

Evaluating platelet flow, the unsuitable rate for clumped platelets accounts for 6.67%, and giant platelets accounts for 10%. Completely resolved in 2019 (0%).Phân tích kết quả ngoại kiểm phết máu ngoại biên (PMNB) từ đó đưa ra các thông số có tỷ lệ đạt thấp hay có nguy cơ sai số tiềm ẩn, khắc phục và đánh giá hiệu suất phòng xét nghiệm (PXN). Đối tượng và phương pháp: Hồi cứu mô tả cắt ngang, có can thiệp/điều chỉnh. PXN nhận mẫu ngoại kiểm dạng PMNB từ Trung tâm kiểm chuẩn Chất lượng xét nghiệm y học - Đại học Y Dược thành phố Hồ Chí Minh mỗi năm 6 đợt, mỗi đợt 2 mẫu lame phết máu từ 2019 đến 2023, thực hiện phân tích và gửi kết quả về đơn vị ngoại kiểm, bảng đánh giá kết quả mỗi đợt được gửi lại sau 10 ngày. PXN xem xét, đánh giá k ết quả không phù hợp hay chấp nhận nhưng cần theo dõi, tìm nguyên nhân ảnh hưởng, đề ra hành động can thiệp để khắc phục sai số, theo dõi đánh giá hiệu suất sau khi khắc phục qua các năm tiếp theo. Kết quả: Kết quả xếp loại PXN luôn ở mức cao với tỷ lệ “rất tốt” tăng từ 66,67%-100%

tỷ lệ xếp loại “đạt” được khắc phục hoàn toàn từ 16,67%-0%. Đánh giá dòng bạch cầu, tỷ lệ bách phân sai lệch ở lymphocyte chiếm 33,33%, neutrophil chiếm 16,67%. Đánh giá dòng hồng cầu, tỷ lệ không phù hợp đối với hồng cầu hình oval chiếm 3,33%, hồng cầu hình bia chiếm 6,67%, mảnh vỡ hồng cầu chiếm 10%. Đánh giá dòng tiểu cầu, tỷ lệ không phù hợp đối với tiểu cầu kết chụm chiếm 6,67%, tiểu cầu khổng lồ chiếm 10%. Kết luận: Sau can thiệp, tỷ lệ xếp loại “rất tốt” tăng cao (tăng thêm 33,34%), tỷ lệ xếp loại “tốt” giảm dần (còn 0%), tỷ lệ xếp loại “đạt” được khắc phục hoàn toàn ở năm 2019 (còn 0%).
