Describe the clinical and paraclinical characteristics of four patients who had prolonged APTT alone while being treated for psycosis with long-term Chlopromazine (group Phenothiazine). Method: Several case reports. Results: Four patients (3 females, 1 male) have been treated by Chlopromazine for a duration ranging from 5 to past 33 years, without any symtoms of anormal bleeding. The APTT ratio ranging from 1.87 to 3.14 but the PT% and fibrinogen concentration are normal or high limits. The Rotem test with CTIntem, CTExtem and CTFibtem is usually prolonged while the Rotem amplitude is normal. The Lupus anticoagulation screening test was positive or not clear. The Lupus anticoagulation confirm test are positive at only in 1 of 3 people, he had results of Anti- Cardiopin IgG positive. Quantification of factor VIIIc, factor IX and factor XI is within normal range. The intervention surgery does not cause unusual bleeding at 2 patients. Conclusion: the prolonged APTT test in patients undergoing long-term treatment with Chlopromazine does not cause spontaneous abnormal bleeding or bleeding during surgical intervention.Mô tả đặc điểm lâm sàng, cận lâm sàng của bốn người bệnh có xét nghiệm APTT kéo dài, kháng đông nội sinh dương tính khi điều trị rối loạn tâm thần bằng Chlopromazine lâu dài. Đối tượng và phương pháp nghiên cứu: Mô tả loạt ca bệnh lâm sàng. Kết quả: Bốn người bệnh (3 nữ, 1 nam) được điều trị rối loạn tâm thần bằng thuốc Chlopromazine thuộc nhóm Phenothiazine từ 5 năm đến 33 năm, không có triệu chứng chảy máu tự phát, xét nghiệm APTT kéo dài, tỷ lệ APTT bệnh/chứng từ 1,87 đến 3,14. Các xét nghiệm PT% và Fibrinogen trong giới hạn bình thường hoặc cao. Xét nghiệm Rotem có CTIntem, CTExtem, CTFibtem thường kéo dài trong khi biên độ Rotem bình thường. Xét nghiệm kháng đông nội sinh không phụ thuộc thời giannhiệt độ của 4 người bệnh dương tính. Xét nghiệm khẳng định kháng đông Lupus và Anti- Cardiolipin chỉ dương tính ở 1 trong 3 người được làm xét nghiệm. Định lượng yếu tố VIIIc, yếu tố IX, yếu tố XI trong giới hạn bình thường. Hai người được phẫu thuật không chảy máu bất thường. Kết luận: xét nghiệm APTT kéo dài, kháng đông nội sinh dương tính ở người bệnh điều trị lâu dài Chlopromazine nhưng không gây chảy máu bất thường tự phát hoặc khi can thiệp phẫu thuật.
