Objective: Accessement of diagnosis, management, outcome of opposite hemorrhagic contusions on patients operated traumatic epidural hematoma. Method: Retrospective study of 38 patients with opposite cerebral hemorrhagic contusions who were operated traumatic epidural hematoma, admitted Viet-Duc hospital between 2/2009 and 2/2011. Results: 55.3 percent contusions discovered before operation of epidural hematomas. Clinic features of opposite lesions on patients operated epidural hematomas: neurological deterioration 23.7 percent, pupillary asymmetry 18.4 percent, elevated ICP 18.4 percent. 47.4 percent patients required surgery (and of these 88 percent were evacuated within 3 days of first: operation) and 52.6 percent non operative treatment. Conclusion: Diagnosis of opposite contusions on patients operated traumatic epidural hematoma base on decreasing GCS, pupillary anomalities, increased ICP. Only 47.4 percent cases need to be operated and 55.3 percent patients get into good outcome.
