Objective: Tissue Doppler Imaging (TDI) may be used for indirect inspection of the main coronary arteries for detection of coronary stenosis and occlusion. This study aimed to assess the ability of TDI to diagnose stenosis in the right coronary arteries (RCA). Subjects and methods: 135 patients, who underwent both TDI, and conventional coronary angiography (CCA), were recruited in this study. The sample was divided into two groups, who were with ( 50 percent diameter stenosis, n = 53 patients) and without significant anatomical stenosis (stenosed RCA s 50 percent or non stenosed, n = 82 patients). Results: The patients with the stenosed RCA had significantly reduced peak systolic (5m) and peak early diastolic (Em) compared to non-case group. Data also indicated that all 5m3, Em3, 5m4, Em4, 5m9, Em9, and 5m 10, Em 10 were an indicator for predicting significant RCA stenosis. Multivariate analysis revealed that the impaired Sm and Em velocities were significantly associated with stenosed RCA. ROC analysis showed that the values of 5.9 cm/s for 5m3 (sensitivity 71.7 percent, specificity 74.4 percent, area under the curve 0.75), and 5.07 cm/s for 5m9 (sensitivity 66.0 percent, specificity 78.0 percent, area under the curve 0.75) could be the reasonable cutoff points to predict RCA stenosis. Conclusion: TDI helps to identify patients with significant stenosis of the RCA and could be used as the screening tool before coronary angiography.
