A six year old girl with diagnosis of lung abscess caused by Paragonimus ringer. This patient presented with a 7 day history of fever, cough and chest pain. On admission, lung examination revealed decreased breath sound on the left without difficulty in breathing. An abscess was diagnosed and chocolate - like fluid was drained from the abscess. The eosinophil count in wac was always higher than normal, and ranged from 11.7 to 43.3 percent. The patient was treated with antibiotics Vancomycin and Amikacin, but the high fever did not ameliorate. The patient was checked for a parasite in all samples of blood, abscess fluid, urine and stool. Finally, she was diagnosed positive with Paragonimus ringer (lgM positive). She was then treated with Praziquantel and her condition improved. She was discharged after 7 days of treatment. the authors recommend colleagues to check for parasitic disease when patients have an eosinophil count that is higher than normal and show no improvement with antibiotics.
