Dorsalis pedis flap (DPF) is one of the plastic material used to cover the surface defect in the foot and ankle territory. The study was to evaluate the result of using the DPF accompany vascular axis in reconstruction the surface defect in the foot and ankle territory. Results show that the size of the flap from 2.5 x 3.5 cm to 8 x 10 cm. Of the 10 studied DPF, there are 6 flaps accompany with proximal vascular and 4 flaps accompany with distal vascular, one flap accompany with distal vascular was necrosis after three weeks late. All of the territory where DPF was taken removal have to be skin grafted. Quality of DPF satisfy functionally and aesthetically at the reconstructive territory. In conclusion, DPF is the appropriate choice for the medium size complicated surface defect in the foot and ankle territory when there is no indication of other nearby flap.
