Purpose: to evaluate the safety and anti-inflamatory effect of Lotemax 0.5 percent (Loteprednol etabonate) in some hyphemas due to contusion. Subjects and method: a prospective clinical trial study was conducted on 24 eyes of 24 patients with hyphema due to contusion. After asking about history of eye trauma, comprehensive eye examin to assessed of visual acuty, IOP, pain, blurred vision, glare, eyelid condition, conjunctiva, cornea, grading of hyphema and associated damages ..., the patients were used to drop Lotemax 0.5 percent 4-5 times / day in the eye irljury for 15 days (2 weeks), addition Atropine 0.5 percent 2 times / day for 5 days. All above index were re-assessed at . all study visites (days 1,3,7 and 15). Adverse events were assessed at all follow-up visits. Results: at the first examinationn, up to 95.8 percent of patients had painful eye, 100 percent of patients felt blurred vision and 83.3 percent of patients felt hyperalgesia in the eyelid. There are a decrease in the order of 8.3 percent, 4.2 percent, 4.2 percent at 3 days after treatment (this difference was statistically significant with p 0.05) and stable at the time of 1 week and 2 weeks after treatment (this difference was not statistically significant with p 0.05). Similarly, at the time of initial examination, up to 83.3 percent of patients had swelling and hematoma in the eyelid, 100 percent of patients had corljunctival congestion and 75 percent of patients had corneal edema. After 3 days of treatment, no patient had corneal edema, however there were still 50 percent of patients had swelling and hematoma of eyelid, 45.8 percent of patients had conjuntival congestion. Almost of all symptoms were completely disappeared at the time of 1 week and 2 weeks after treatment with Lotemax. In the study, no patient had unwanted side effects. Conclusion: Lotemax is effective and safe drug for treating anti-inflammatory after hyphema due to contusion. However, futher studies are needed.
