Background: Splenic injury is common in abdominal trauma (20-30 percent). The splenic preservation in trauma is presennted but nonoperative management isn't popular in Viettiep hospital. Purpose: Evaluate the result of nonoperative management of splenic injury. Methods: Retrospective and prospective study on 41 cases of non-operative management of splenic injury at Viettiep hospital from 2008 to 2011. Informations gathering: principle characters, clinic and paraclinical signs and result of therapy. Result: The mean age was 25 percent:14, sex ratio was 3/1, the main cause was tranport accident (58,5 percent). 92,7 percent of patients were hemodynamically stable at hospi~alization while 7.3 percent of cases have hemodynamic stablelity after 24hrs of reanimation. 87.8 percent of cases have direct signs on ultrasound who are the laceration, the parenchymal injury. The successful rate of nonoperative management is 97,8 percent for grade I-IV, 100 percent for grade I-III, 67 percent for grade IV (AAST grade scale), therapy time was 9.78 days. Conclusion: Non-operative management of splenic injury in abdominal trauma is considered the standard therapy while the patient's hemodynamic is stable.
