 Severe liver injury was the liver damage of grade IV or higher - by the AAST. With this grade of liver lesions, the treabnent depends on the patient's hemodinamical status at admission and combined lesions. Therefore, this study carried out with the aim of assessing the treabnents of severe liver damage due to blunt abdominal trauma. Results: from 01/2007 to 12/2009 Viet Duc hospital in Hanoi, there were 87 patients under dosed hepatic injury with the lesions of grade IV or higher that were treated, of which were 66 males (75.9 percent), 21 females (24.1 percent), the age averaged 29.9 + or - 13.4 (ranges from 7 to 81 years old). The cause of injuries were 71 traffic accidents (81.6 percent), 13 working accidents (14.9 percent), 3 living addents (3.4 percent). The mean time of admission from accidents was 23.2 + or - 14.5 hours (range from 30 minutes to 120 minutes). The admission of mean heart rate was 94.9 + or - 15.8 per minute, maximum systolic blood pressure was 104.4 + or - 3.8 mmHg
  the red cells were 19.3 + or - 0.8 million/mm3, Hemoglobin was 109.4 + or - 24.3 g/I, Hematocrit was 31.9 + or - 6.7 percent. 17 the patient must transfuse to stabilize hemodinamically. Diagnosis and classification of hepatic lesions were ronducted through abdominal computed tomography scanner or through the description of hepatic lesions during urgent surgery. Results: operative treabnent was 13 cases (14.9 percent) for emergency, nonoperative management was 74 cases (85.1 percent)
  complications of urgent operative group were 2 cases (2.3 percent), of conservative group were 18 cases (20.7 percent)
  mortality was 1 case (1.1 percent). The hospitalization time averaged 12.4 + or - 7.4 days (ranging from 2-37)
  98.1 percent of patients discharged.
