Persistant pulmonary hypertension of the newborn (PPHN) with right to left shunt occurs in variety of clinical situations. Newborn with persistant pulmonary hypertention are exceptionally unstable and difficult to manage. There are three main types: PPHN with normal lung on X-ray (idiopathic PPHN), PPHN associated with pulmonary parenchymal disease, PPHN associated with hypoplastic lung tissue. The authors present a case PPHN with normal lung on X-ray who was managed by iNO. Baby recoved well after 2 days on iNO, arterial blood pressure was stable. And the authors suggest to use iNO early in patient with idiopathic PPHN.
