Objectives: The goal of this work is aimed to evaluate the dinical presentation and the coagulation disorders in spontanous intracerebral hemorrhages (SIH). Methods: This study was conducted in all patient with spontanous intracerebral hemorrhages admitted to Department of Neurosurgery and Spinal surgery of Binh Dinh general Hospital from 5/2009 to 5/2010. Age, sex, clinical manifestation, hemostatic parameters of coagulation including the platelet count, prothrombin time, activated partial thromboplastintime (PTT) follow-up to evaluate the intrinsic pathway and the extrinsic pathway of coagulation, early results of treatment, complication were analysed. Results: There were 170 patients: 90 male (52,9 percent) and 80 female (47,1 percent), mean age was 62,52 :t 1,32 year-old. The most common type of sIH were lenticulostriate in 58 cases (34,12 percent), lobar in 56 cases (32,94 percent), intraventriculare 34 cases (48,57 percent), subarachnoid 11 cases (6,47 percent). Coagulation disorders followed-up spontanous intracerebral hemorrhages in 65 cases (38,2 percent). The diffrence of he frequency of coagulation disturbances in the various type of SIH were statistically no significant. The disfunction of the extrinsic pathway occurred in 33 cases (17,65 percent), intrinsic pathway were in 14 cases (8,24 percent), mixed 9 cases (5,29 percent), decrease platelet count was in 9 cases (5,29 percent). Medical history of arterial hypertension appeared to be relacted factor with coagulation disorder in spontanous intracerebral hemorrhages and the coagulation disorder associate more frequent in the poor results than others. Surgical lesions were in 12 cases (7,1 percent), good result were in 87 cases (51,2 percent), morbility was in 24,7 percent and mortality in 24,1 percent. Conclusion: Clinical presentation of spontanous intracerebral hemorrhages were complicated. The coagulation disorder occurred in 38,2 percent cases. The most frequent kind of coagulation disturbances were disfunction of extrinsic pathway in 33 cases (17,65 percent), of intrinsic pathway in 14 cases (8,24 percent), mix in 9 cases (5,4 percent), decreased platelet count in 9 cases (5,4 percent). The coagulation disorder occured more common, in the patient with medical history of arterial hypertension than the others and seemed to be a poor prognostic factor.
