Introduction: Pancreatic cysts are common, most cysts are largely asymptomatic and range from benign to premalignant to maglignant cysts. Diagnosing the nature of these cysts is important but is difficult. Material and method: Study reprospective of 22 patients operated by neoplastic cysts in Viet Duc hospital in 2 years 2010-2011 consisting of 18 women (81.8 percent) and 4 men (18.2 percent). Results: Treatment: 18 pancreatectomies (81.8 percent) (4 duodenopacreatectomies, 14 resections of corp and tail), 3 pancreatojejunostomies (13.6 percent), 1 cystic drainage (4.5 percent). Pathology: 10 serous cystic neoplasm (45.5 percent), 6 solid pseudopapillary neoplasm (27.3 percent), 3 mucinous cystic neoplasm (13.6 percent), 2 cystic neuroendocrine tumors (9 percent), 1 cystadeocarcinoma (4.5 percent). Conclusion: Pancreatic cysts neoplasm are difficult to differentiate between benign and malignant cysts. Exclude the small, asymptom serous cysts neoplasm could survey, the others cysts neoplasm are recommended to be removed as possible. Imaging diagnostic modalities (echography, cr, MRI, echo-endoscopy...) are of important role in diagnosis and management.
