Objective: To evaluate the outcome of alveolar cleft bone graft and the success of implant. Subject and method: The prospective uncontrolled clinical trial research performed on 32 patients with complete unilateral cleft lip and alveolar cleft defect, including 23 female and 9 male in the age from 15 to 29 years. Result: The mean apical-coronal distance of the bone graft was 11.4 +/- 2.4mm and the mean buccal-lingual distance was 6.l +/- 1.0 mm. 29 patients (90.6 percent) were rated type I on Enermark scale enabled to place implant, 3 patients (9.4 percent) type III had insufficient bone for implant placement required the fix bridge restoration. Total of 32 implants were used, the implant survival rate was 100 percent in the follow-up period of 18 months. 100 percent patients satisfied with their implant restoration and 71.9 percent implant prostheses reached the threshold of clinical acceptability and higher. Conclusion: The two iliac bone block auto-graft for implant placement for cleft lip palate patients is a realistic and feasible solution that not only meet the chewing function but also reach aesthetic improvements.
