Objective: Evaluation of results of using free flaps for reconstruction in Department of Maxillofacial and Plastic Surgery. Subject and method: between 1990 and 2014, 654 free flaps were harvested in 630 patients with mean age 34.5 years old, underwent microsurgical reconstruction in Department of Maxillofacial and Plastic Surgery, 108 Military Central Hospital. Result: The total flap survival rate was 92.20 percent, partial flap necrosis 5.35 percent and total flap necrosis 2.14 percent. The functional and aesthetic outcome of some individual flaps: In group of functional muscle transfers was 85 percent, in group of flaps for surface reconstruction was 82 percent in group of vascular bone flaps was 92 percent. Conclusion: From the experience, the authors suggest that the recent advances in micro technique and free flap studies have provided rarely useful toots for improving in general and plastic and reconstructive surgery in particular.
