BACKGROUND: Atherosclerotic cardiovascular disease (ASCVD) is one of the biggest threats to public health worldwide. It shows a trend of high incidence rate and younger age. Abnormal lipid metabolism is the main risk factor. The residual risk of remnant cholesterol (RC) has been studied recently. However, the distribution of remnant cholesterol (RC) with gender and age is rarely reported. METHODS: Data were collected from the laboratory information system from September 1, 2022, through March 31, 2023. TC, HDL-C, and LDL-C were measured using a Roche cobas 8000 c701 automatic biochemical analyzer. RC was calculated using Sampson formula. The median, 75th percentile, and 25th percentile were used to describe the distributions and were presented using scatter plots of relevant analytes among total subjects, normal cholesterol, and abnormal cholesterol subjects. Gender and age differences were compared using SPSS.25 soft-ware for RC. RESULTS: A total of 20,265 records of individuals were enrolled in this study after ineligible records were removed. The TC, LDL-C, and non-HDL-C concentrations showed peaks in total subjects and females aged 51 - 60 years. Meanwhile the peaks in males were found in males aged 31 - 40 years. The HDL-C concentration showed peaks in females aged 21 - 30 years and in males aged 2 - 20 years. There were lower RC concentrations in females aged 21 - 40 years and in males aged 71 - 90 years. The differences were significant between different genders and different ages. CONCLUSIONS: The distribution trend of RC in the population was different from that of the common cholesterol, which means the management of RC in the population should be stratified by gender and age.
