Uveal melanoma is a rare and highly malignant tumor. Its occurrence in children is rare and challenging to differentiate from retinoblastoma. The present work reports on the unusual case of a 4-year-old male patient who presented a mass in the right eye unresponsive to 18 days of ophthalmic artery chemotherapy. Enucleation revealed a solid black mass with high melanin content. The pathological diagnosis indicated uveal melanoma in the right eyeball, infiltrating the cornea and sclera. Whole-exome sequencing found no pathogenic gene variants. Further exploration of pathogenesis and molecular genetics is needed.
